Dark Lips? Here’s How Diode Laser Therapy Is Changing the Game

Dark or discoloured lips are one of the most common cosmetic concerns we see at the clinic. For many patients, it isn’t caused by any disease — it’s simply the way their skin naturally pigments. Yet the impact on confidence can be significant, and until recently, treatment options have been frustratingly limited.

That’s why we’re excited to share the findings of our own pilot study, recently published in Pigment International, exploring a new approach: diode laser therapy for physiological lip hyperpigmentation.

Why Do Lips Get Dark in the First Place?

Lips naturally contain a much higher concentration of pigment-producing cells (melanocytes) relative to skin cells than most other parts of the body. This ratio is part of why lips can appear darker in many individuals, particularly those with deeper skin tones (Fitzpatrick skin types IV, V, and VI). Interestingly, research shows that people of different ethnic backgrounds don’t actually have different numbers of melanocytes — the difference comes down to how much pigment those cells produce and how it’s distributed within the surrounding skin cells.

In most cases, this kind of pigmentation is completely harmless and needs no medical treatment at all. But for patients who want a cosmetic improvement, options have traditionally been underwhelming: depigmenting creams, chemical peels, and Q-switched Nd:YAG laser have all been tried, with inconsistent or slow results and often multiple, costly sessions required.

An Accidental Discovery

Our interest in diode laser for this purpose actually began by chance. While treating a patient for upper lip hair reduction, we noticed that a small area of her lip lightened unexpectedly after the session — a known but uncommon side effect of diode lasers on pigmented skin. Rather than see it purely as a complication, we recognised its potential and decided to study it properly.

What We Did

We designed a pilot study involving 12 patients with physiological lip hyperpigmentation who had not responded to other treatments in the past. Each patient received a diode laser session (Lumenis LightSheer, an FDA-certified device) at the outset, with a possible repeat session after two months if pigmentation remained. Patients were followed monthly with photographs over a total period of six months, and their improvement was graded using a standardised pigmentation and response scale.

What We Found

The results were genuinely encouraging:

  • Over 90% of patients showed a good to excellent improvement in lip pigmentation by the end of six months.
  • Half of the patients achieved an excellent response — more than 75% clearance of pigmentation.
  • Most patients needed just a single session; only a few required a second treatment for residual, patchy darkening.
  • All patients reported satisfaction, with the majority describing themselves as satisfied and a smaller group as extremely satisfied.
  • Side effects were mild and temporary — some redness, swelling, and light crusting that resolved within one to two weeks, with no lasting complications or recurrence during follow-up.

We also observed something clinically interesting: patients with darker, more pigmented lips at baseline tended to respond even better than those with lighter pigmentation.

Why This Matters

Compared to chemical peels or Q-switched Nd:YAG laser — which often require multiple sessions with variable outcomes — diode laser therapy offered faster, more predictable results in our small study group, often in just one sitting. For patients who have tried other treatments without success, this represents a promising new option.

Of course, as a pilot study with a limited number of patients, larger studies are needed to confirm these findings and refine treatment protocols. But the early results give us real confidence in offering this as a treatment option at our clinic.
