Scar Wars: How Pinhole CO₂ Laser Ablation Is Changing the Game for Papular Acne Scars

If you’ve ever looked in the mirror and noticed small, raised, skin-colored bumps left behind after acne has cleared — especially across your nose, cheeks, chin, or jawline — you may be dealing with something that’s often overlooked in conversations about acne scarring: papular acne scars.

Most people are familiar with the “textbook” acne scars — icepick, boxcar, and rolling scars, which sit below the skin’s surface. Papular scars are different. They’re small, soft, elevated bumps, usually just 1–4 mm wide, that sit slightly above the skin, giving the skin a subtle cobblestone-like texture. Because they aren’t as dramatic-looking as pitted scars, they’re frequently under-recognized — but they can be just as frustrating to live with, and just as resistant to standard treatment.

Why Are Papular Scars So Hard to Treat?

Papular acne scars form when inflammation from acne damages the collagen and elastin fibers around hair follicles, leading to areas of superficial scar tissue, mild swelling, and small dilated blood vessels under the skin. Unlike atrophic scars, they don’t usually respond well to oral antibiotics, isotretinoin, or other standard acne medications.

Traditional options — chemical peels, dermabrasion, radiofrequency electrocoagulation, and even standard ablative lasers — have been used with mixed results, and they carry a real risk of side effects like post-inflammatory hyperpigmentation, especially in darker skin types.

Our Study: Putting Pinhole CO₂ Ablation to the Test

We recently published our findings in the Journal of Cutaneous and Aesthetic Surgery, based on a retrospective study of 65 patients with papular acne scars treated using super-pulse pinhole CO₂ laser ablation. Here’s what the technique involves and what we found.

How the Pinhole Technique Works

Rather than resurfacing the whole treatment area the way traditional ablative lasers do, the pinhole method delivers highly focused super-pulse CO₂ laser energy directly into each individual papular scar through tiny perforations — essentially treating each bump precisely, while sparing the surrounding healthy skin. This targeted approach allows for controlled remodeling of the scar tissue and encourages the skin to regenerate more even, aligned collagen, without the broader epidermal damage associated with conventional lasers.

In our protocol, patients received 2–3 sessions spaced 4–6 weeks apart, with numbing cream applied beforehand and simple aftercare (antibiotic ointment, sunscreen, and a short course of topical/oral antibiotics) afterward.

What We Found

Our patients — average age 31, with scarring most commonly affecting the cheeks and nose — showed encouraging results:

• Over 80% of patients showed significant clinical improvement, with a real reduction in scar height and contour irregularity.
• Improvement continued to build over time — by the 3-month follow-up, more than half of patients had achieved 51–75% or greater improvement based on physician evaluation.
• Side effects were minor and short-lived — mainly temporary redness and swelling.
• No cases of post-inflammatory hyperpigmentation or new scarring were observed — a particularly important finding for patients with medium-to-darker skin tones (the majority of our study population had Fitzpatrick skin types IV and V).
• Follow-up compliance was excellent, with 98.5% of patients returning for their post-treatment evaluations.

What This Means for Patients

Papular acne scars often get dismissed as “just texture,” but they can meaningfully affect confidence and skin appearance. What our data suggests is that pinhole CO₂ ablation offers a precise, low-downtime option that specifically targets these raised scars — without the pigmentation risks that have historically limited laser resurfacing for this type of scarring.

Because the technique treats each scar individually rather than resurfacing the whole face, it also tends to involve less discomfort and a shorter recovery period than more aggressive laser treatments.
